Comments: This patient had a convex nasal dorsum and her nasal tip was rotated downward. This procedure provided a more aesthetic dorsal profile and lifted the tip. The view from below shows the more refined nasal tip. She had the surgery after graduating from high school and before starting college in another state. She thought this would be a good time to do it, before meeting a lot of new people for the first time.

Comments: This patient has always had trouble breathing through the right side of her nose. This was corrected with a septoplasty. Simultaneously, the appearance of the nose was improved with a rhinoplasty, which involved removing the bony hump, narrowing the nasal bones, and trimming the “alar” cartilages of the tip of the nose. Her “after pictures” were taken only 3 weeks after surgery. She still has swelling of the tip. She was excited to be able to breathe normally through her nose for the first time.

Comments: This woman did not like the "beak-like" appearance of her nose and also felt it was too large for her face. The dorsal hump was corrected with rhinoplasty.

Comments: This patient had longstanding nasal obstruction, severe enough that she had a hyponasal voice quality, sounding like she always had a cold. She desired a smaller, prettier nose. A septorhinoplasty was done, relieving the nasal obstruction, allowing her to breathe normally through her nose, and improving her appearance. Often, such patients have not breathed normally before so they don’t know what they are missing until the obstruction is relieved.

Comments: This patient had long wanted to reduce the size of her nose. She had a dorsal hump and over-projecting tip, which were reduced with a “closed” rhinoplasty (incisions only on the inside of the nose). Her chin was augmented to improve her profile, correcting the negative synergy of a large nose and small chin. Liposuction was performed simultaneously under the chin. Before surgery, she was unsure whether to have the chin augmentation in addition to the rhinoplasty. After surgery, she was as happy with her chin as she was with her nose. She was back working as a nurse one week after surgery.

Comments: This 26 year-old nurse had a long nose with a reduced “nasolabial angle,” closer to the 90 degrees that typifies a masculine nose. The classical feminine nose has a slightly upturned nasal tip. This procedure shortened the nose, providing elevation of the nasal tip and a more feminine appearance.

Comments: This patient was unhappy with her dorsal hump and had trouble breathing through her nose. She had a deviated septum and enlarged turbinates. The septal obstruction was eliminated and the turbinates were trimmed. A rhinoplasty was performed simultaneously to improve the appearance of her nose. Note that the projection of her tip has been reduced, evident on the “worm’s eye” view. Ideally, there is no sign of surgery and the nose does not look as if it’s been operated on.

Comments: This Mexican-American female did not like her “beaky” nose. The dorsum was reduced, a supratip depression created to eliminate the convexity, and the tip cartilages were trimmed. This provided a more refined appearance of her tip. She has an excellent jawline and her profile is now complimented by her nose.

Comments: This patient was dissatisfied with the result of a rhinoplasty done elsewhere two years previously. She had a dorsal irregularity and a depressed nasal tip. The small bump was removed and the tip reconstructed with cartilage graft taken from the septum, inside the nose.

Comments: This patient thought a rhinoplasty might help her modeling career. She had a large, overly-projecting, boxy nasal tip. The alar cartilages were reduced. She subsequently had fat injection of the lips, apparent in the “after” pictures. Even at the age of 40, she is enjoying working as a model.

Comments: This patient thought she looked “like a cross between Cameron Diaz and Kirsten Dunst.” But she was unhappy with her bulbous nasal tip. This was corrected with a tip rhinoplasty.

Description: Asian Female Rhinoplasty:Septorhinoplasty with Septal and Conchal Cartilage Grafts, Nasal Base Reduction and Chin Augmentation.

Comments: This 36 year-old woman of Malaysian ancestry wanted a more westernized look. Grafts were taken from the septum and ear to help build up the dorsum and tip of the nose (see Intraoperative Photos). A chin augmentation balanced her profile. The bases of the nostrils were shortened to reduce flaring. The bridge of the nose was built up and the tip reconstructed to correct her tip depression and provide a more western look without overdoing it or making her look “operated on.” Her breathing was improved as a result of the septoplasty, which served dual purpose as a donor site for cartilage.

Comments: This patient sustained injuries to her nose and upper lip in a fall onto a sidewalk the year before. Her nasal deformity consisted of a dorsal hump, depressed nasal tip, no supratip depression (the pleasing break just above the tip of the nose), and a reduced nasolabial angle. She also had a scar contracture of the upper lip, pulling up on the right side of her lip and causing a conspicuous deformity. She had a reddened area on the right side of the chin, left over from an abrasion, which was also treated with the pulsed dye laser.

Comments: This patient had a previous rhinoplasty elsewhere. She presented with a severe “saddlebag” deformity with collapse of the nasal tip and a large hole (“perforation”) in her septum. This deformity also occurs in cocaine abusers (not this patient) because cocaine is a potent vasoconstrictor and interferes with the blood supply to the septal cartilage. Cartilage from both ears was used to rebuild the tip cartilages with a result that was acceptable to the patient and important professionally, although she still has some asymmetry and it was not possible to repair her septum.

Comments: This 41 year-old actress wanted a more Caucasian nose and brought along pictures of Halle Berry. She elected to have a chin/jowl augmentation too. An open approach was used (note the columellar scar). Her nasal projection was increased using a cartilage graft to the columella, the tip refined, and the flaring of the nostrils was reduced. These maneuvers are typical for the non-Caucasian rhinoplasty. She still has some nasal tip swelling in these pictures. The patient was crying as I entered the exam room at the time of her postoperative visit. To my relief, these were happy tears.

Comments: This Mexican-American woman said she was always embarrassed by the appearance of her nose and was subject to teasing about it when she was growing up. She described it as “banana-shaped.” She also had nasal obstruction and took medication daily for this. She had large turbinates causing airway obstruction. She wanted a big difference, but at the same time wanted her nose to fit her ethnic background. She said her sisters and friends were very complimentary about the result. After surgery, she was able to discontinue her nasal spray.

Comments: This young patient, about to start law school, had longstanding nasal obstruction from a severely deviated nasal septum. He also had a large dorsal hump. A conspicuous depressed scar of the left temple was revised simultaneously. The “worm’s eye” photographs show a reduction in projection of the nose (which is how far the tip sticks out from the face). The shape of the nose when seen from below approximates the desired equilateral triangle instead of an isosceles triangle. The nostrils appear smaller and rounder even though there has been no removal of skin from the circumference. This result is due to the reduction in the cartilage framework and skin contraction.

Comments: This patient wanted his nose reduced. He had a prominent dorsal hump, full nasal tip, decreased nasolabial angle, and moderate alar flaring. A closed approach was used. The nasolabial angle was corrected to the masculine ideal of about 90 degrees. He would also have benefited from a chin implant.

Comments: This patient was very articulate in describing what he did not like about his nose – the nose was too large, too pointed, asymmetrical, with a mild dorsal hump. He was a soccer player and thought he may have fractured his nose on a few occasions. He brought along pictures of soccer star David Beckham to give me some idea of his desired appearance. The patient was very pleased with the result and had also received compliments from his girlfriend. This result shows that only a few millimeters difference in nasal contour produces a noticeable improvement in appearance and can be very gratifying for patients.

Comments: This 34 year-old soldier said he’d jumped out of planes and been shot at, but was never as nervous as he was on the day of surgery. An old scar of the left jawline was revised simultaneously. He did not find it necessary to take the prescribed painkillers and took Tylenol® instead. Eight days after surgery he was back in uniform. At the time of his visit nine days after surgery, he still had the expected degree of swelling and bruising. The brown spot on the right cheek was gone. When he returned home for Thanksgiving, he said only one of his two sisters noticed a difference in his appearance. His mother did not. He was told that he looked younger. He said that he ‘could not be happier” with the result.

Comments: This 37 year-old Hispanic male had always been self-conscious about his small chin and wore a goatee for camouflage, like so many men who have a small chin. He wanted a more Caucasian appearance.

Testimonial: "I had some breathing problems because of past sports injuries to my nose and I decided to have my upper eyelids done at the same time. The first two or three days were pretty miserable. After that, it really wasn’t bad. The difference in my appearance is subtle and natural-looking, which is what I wanted, and my breathing is better.”

Comments: This patient wanted a larger, more “Roman” nose. He also had nasal obstruction from a deviated septum. The septoplasty served two purposes – correction of the nasal obstruction and graft material to be used, along with cartilage from the left ear, to build up the nasal dorsum. His “bug-eyed” appearance was treated simultaneously with lower blepharoplasties and fat injection to the upper cheeks and tear troughs. His upper lip and nasolabial creases were also treated to give him a fuller lip and smooth his smile lines. He attended church 10 days after surgery and was back to work servicing heating and air conditioning systems 11 days after surgery.

Comments: This patient thought his face looked “pudgy.” He was treated with a combination of a tip rhinoplasty, submental lipectomy, chin/jowl augmentation and resection of his cheek fat pads. This helped give him a more chiseled profile.

Comments: This patient found his nasal tip a little too bulbous and droopy. He is seen only one month after his rhinoplasty (he still has some swelling of the tip). The tip of the nose is elevated so that the angle between his nose and upper lip is now 90 degrees, the preferred angle for men. His nose looks more refined, but not overly so. He reported very little pain and in fact did not take any painkillers after the date of surgery. He has limited swelling and bruising and was comfortable attending a wedding just a few weeks after his surgery. He was also glad that I had not packed his nose and that he could breathe through his nose easily the day after surgery.
