Eyelid Rejuvenation - BEFORE AND AFTER PHOTOS

Comments: This Korean-American patient requested a more western look. She had trouble applying makeup. The epicanthal skin folds rested on her eyelashes, deflecting them down. These folds were released using small interpositional skin flaps. The caruncles are now visible in the inner corner of her eyes, which appear more open. She had no trouble afterward with eye makeup. Her sutures were removed four days after surgery and she returned to work with makeup six days after surgery.

Comments: This 30 year-old had severe dark circles which made her look tired and forced her to “cake on” makeup to try to hide them. Amazingly, even her two year-old daughter had dark circles. Because of the severity of her condition, the combined three-pronged treatment was recommended. The laser was used with lower blepharoplasties and fat injection into the tear troughs (the creases under the lower lid bags) to help lighten the skin, smooth premature wrinkles, and lighten her dark freckles. Although the dark circles were not completely eliminated, there was substantial improvement. She no longer felt like she was a “prisoner of makeup” after surgery.

Comments: This patient’s baggy lower eyelids were treated with transconjunctival blepharoplasties and laser resurfacing. Because she had minimal skin redundancy of the lower lids, this approach was used, concealing the incision on the inside of the lower lid. She is seen only 10 days after surgery. There is still some redness from the laser treatment. She returned a few years later to have her upper lids treated.

Comments: This patient disliked the bagginess of her eyelids and fine wrinkles, particularly her crows’ feet. She also had small skin lesions of the right upper eyelid and orbital rim, to be removed simultaneously. She had folds of the inner corner of the eyelids, called “epicanthal folds,” which are common in patients of Asian descent. This patient was not Asian; she was Romanian on her mother’s side and her father was French-Canadian. These folds were released using small “Z-plasties.” Because of her youth and minimal skin laxity, a transconjunctival approach was used for her lower lids, concealing the incision on the inside of the lid. After her eyelid surgery, she received many compliments. Her fiancé said her beautiful eyes were the first thing he’d noticed about her. Although she was obviously attractive, she said nobody had complimented her eyes before.

Comments: This patient had previous lower blepharoplasties, performed elsewhere. She was concerned about extra skin folds of the upper lids. Accordingly, upper blepharoplasties were recommended. CO2 laser resurfacing was used to provide skin tightening of the lower lids and crow’s feet areas. She had old "chicken pox" scars that were revised simultaneously. She has some redness of the eyelids two and a half weeks after a touchup laser treatment to provide some additional skin tightening.

Comments: This 40 year-old patient has five children. She said that she had looked tired for at least a decade. Although she no doubt had reason to be tired, she did not wish to look it. She had significant past UV exposure from a tanning bed. She had excess fat causing bagginess of the lids. This excess fat was trimmed at surgery.

Comments: This patient had dark circles of her lower lids, along with skin redundancy and bagginess of her eyelids. She had distinct “tear troughs” that made her look older. She wanted the maximum possible improvement. She started with upper and lower blepharoplasties and fat injection. Six weeks later, a combination of CO2 and erbium laser resurfacing was performed. She said people frequently complimented her on her sparkling blue eyes. Before surgery, nobody seemed to notice. She is seen ten months after surgery without makeup.

Comments: This 42 year-old felt she was looking older and wanted to improve the dark circles of her lower eyelids, which she said had been present for years. The “triple combination” of lower blepharoplasties, laser resurfacing around the eyes, and fat injection was used for maximum benefit, making her look younger and more refreshed.

Comments: This patient’s background was Mexican on her father’s side and a mix of Mexican and American Indian on her mother’s side, although she was frequently taken for an Italian. People commented that she looked tired. She was aware that her baggy eyelids made her look older. She was surprised how little discomfort she had after surgery. She said she took one painkiller but didn’t really need it. One month afterward she remarked that sometimes her eyes looked great and then they would swell again. By three months, the swelling had resolved. She thought her eyes looked brighter and she no longer looked tired.

Comments: This patient was a nonsmoker but had noticeable vertical lip lines and dark circles which made her look older than her 46 years. She grew up on the island of Guam and had premature skin aging due to sun exposure. She had surgery in stages, accompanied by body contouring surgery. First, she had fat injection of the lower face and perioral CO2 laser resurfacing, followed the next year by blepharoplasties, additional fat injection, and laser resurfacing, which she found very helpful. She went to Hawaii on vacation two months after surgery but was careful to use sunblock and avoided hyperpigmentation. Her dark circles and lip lines have been corrected.

Comments: This 48 year-old Hispanic patient had dark circles, skin redundancy, and bagginess of her eyelids, making her look weary. She was delighted with her more youthful appearance. She was engaged shortly after her surgery and her fiancé was very complimentary. She did experience a complication - hyperpigmentation (see Complications section in Laser Skin Resurfacing), which responded to sunblock and a bleaching cream.

Comments: This patient had been considering having her eyelids treated for several years. With her 50th birthday approaching, she thought it was time to take action. She thought she looked younger after surgery and received compliments on her appearance. She is seen four days after surgery, immediately after removal of her sutures.

Comments: This patient had typical findings of photoaging in a 50 year-old white female. She had facial wrinkling, a prominent frown line, depressed eyebrows, tired-looking eyelids, and early loss of fat volume. Accordingly, this combined approach was used. Amazingly, she was back to work eight days after surgery, after a makeup session at the office.

Comments: This patient had full facial rejuvenation, producing the same youthful appearance from top to bottom. The improvement in her eyelids is particularly apparent on the oblique photographs. After surgery, the lower eyelid blends in with the cheek, rather than being demarcated by a crease. This is a characteristic of a youthful face. She looks younger without any telltale signs of surgery. These results show that multiple techniques done together can produce natural and optimal results while avoiding an “operated on” look. The tip of her nose was reduced at the same time. Her appearance is not altered, just enhanced.

Comments: This patient decided to have her baggy eyelids treated simultaneously with a tummy tuck and liposuction on her 52nd birthday. Later, laser resurfacing was used to provide additional skin tightening, further improving the skin tone of the lower eyelids.

Comments: This 77 year-old insulin-dependent diabetic female presented with severe dark circles and tear trough deformities. In fact, this is possibly the worst case of dark circles I’ve treated. Two months after surgery she still has some redness. Conspicuous brown spots on the forehead and right cheek have been eliminated. The redness of the upper forehead is due to an existing skin condition.

Comments: This patient did not like his “bug eyes” and wanted a larger, more Roman nose. The area under the eyes was filled in with fat so that his eyes no longer appear to protrude. The lower blepharoplasties eliminated the excess skin and helped to redrape the tissues. Combined with fat injection, this procedure smoothed out the lid-cheek junction, making him look younger. The rhinoplasty has given him a stronger, more masculine nasal profile.

Comments: This 60 year-old school counselor found that the extra loose skin of his upper lids was starting to interfere with his vision. We discussed a combination of upper blepharoplasties, to trim this excess skin, and an endoscopic forehead lift to reelevate his eyebrows. He understood that sagging eyebrows were contributing to the problem. He had his surgery during the summer school break. He found it amusing that one of the teachers asked if he’d been meditating during the summer break. His eyesight was improved immediately. He was especially pleased at how well hidden the incisions were and the fact that he did not look “overly alert.”
