Comments: This 39 year-old diabetic female had extra fatty tissue of the eyelids which made her look tired. She noticed a difference as early as a week after surgery. She encountered hyperpigmentation, a complication of laser resurfacing, which responded to a mild bleaching cream.

Comments: This patient had extensive past sun exposure and owned a tanning bed. She looked considerably older than her 43 years. A number of facial moles were removed by excision (“cutting them out”). Patients often wonder if moles may be removed with a laser. It is usually better to cut them out because they involve the deeper layers of the dermis and this will reduce the risk of recurrence and leave a small line rather than a saucer-shaped depression, which tends to happened after treatment with nonsurgical techniques such as scraping them off (“curettage”), cautery, or cryotherapy with liquid nitrogen. She returned for a facelift two years later.

Comments: The CO2 laser was used to treat most of the face, including the areas of deeper wrinkling around the eyes, lower forehead, and around the mouth. The cheeks were treated with the erbium laser. Blepharoplasties were performed, using the “transcutaneous” skin incision following the eyelash margin (which allows greater tightening of the lower lid skin), along with fat injection and an endoscopic forehead lift. She was excited about her progress even a week after surgery. She went on a ski trip three weeks after surgery. She was careful to use sunblock and avoided hyperpigmentation.

Comments: This patient had extensive photodamage. She was concerned about her premature facial aging, particularly the facial creases. To obtain the most lasting result, CO2 laser resurfacing and facial fat injection were recommended as a combined procedure. A total of 46.5 cc of fat were used. She took two weeks off work. This combination of “plumping from below” and “resurfacing from above” can be very effective for deep skin creases.

Comments: This patient had typical findings of photoaging in a 50-year-old white female. She had facial wrinkling, a prominent frown line, depressed eyebrows, tired-looking eyelids, and early loss of fat volume. Accordingly, this combined approach was used. Remarkably, she was back to work 8 days after surgery, after a makeup session at the office.

Comments: This patient underwent radiation treatments for acne when she was a teenager. She started to develop premature wrinkling around her mouth in her late twenties. Laser resurfacing provided substantial improvement for a problem that had caused her to be self-conscious most of her adult life. Because of the severity of the condition, the CO2 laser was used. She returned to her job as a teacher 3 weeks after surgery, and shared her experience with her fourth graders. She said the response was very positive. Postoperative swelling helps to camouflage her wrinkles, which have partially returned 9 months after surgery. This could be treated with additional laser resurfacing, possibly with a touchup fat injection, although the patient told me she thought the difference was like “night and day” and has not come back.

Comments: This patient had a facelift two years previously. She wanted to smooth her facial wrinkles and eliminate age spots. She returned later for facial fat injection and a forehead lift.

Comments: This patient wanted to look her best on her 60th anniversary. Her eyelids had been treated in the past. She had extensive skin damage due to sun exposure, having spent the winters in Hawaii over a period of decades. In fact, I believe she had the worst photodamage of any patient I have treated! She would also have benefited from a forehead lift, but her face and neck were her priorities. Sun-related skin lesions of the face were removed simultaneously.

Comments: This 81-year old was taking anticoagulants for an artificial mitral valve. Therefore, she was not a candidate for surgical treatment. Fat injection was not an option. She elected to have laser resurfacing and injection of Juvederm to improve her wrinkles.
