R.N. (Age 20) - Student
Procedure: Rhinoplasty
Description: Septorhinoplasty, Excision of Old Chicken Pox Scar of Left Temple, and Excision of Multiple Facial Skin Lesions

Comments: This young patient, about to start law school, had longstanding nasal obstruction from a severely deviated nasal septum. He also had a large dorsal hump. A conspicuous depressed scar of the left temple was revised simultaneously. The “worm’s eye” photographs show a reduction in projection of the nose (which is how far the tip sticks out from the face). The shape of the nose when seen from below approximates the desired equilateral triangle instead of an isosceles triangle. The nostrils appear smaller and rounder even though there has been no removal of skin from the circumference. This result is due to the reduction in the cartilage framework and skin contraction.

Comments: This patient wanted his nose reduced. He had a prominent dorsal hump, full nasal tip, decreased nasolabial angle, and moderate alar flaring. A closed approach was used. The nasolabial angle was corrected to the masculine ideal of about 90 degrees. He would also have benefited from a chin implant.

Comments: This patient was very articulate in describing what he did not like about his nose – the nose was too large, too pointed, asymmetrical, with a mild dorsal hump. He was a soccer player and thought he may have fractured his nose on a few occasions. He brought along pictures of soccer star David Beckham to give me some idea of his desired appearance. The patient was very pleased with the result and had also received compliments from his girlfriend. This result shows that only a few millimeters’ difference in nasal contour produces a noticeable improvement in appearance and can be very gratifying for patients.

M.T. (Age 34) - Armed Forces
Procedure: Rhinoplasty
Description: Rhinoplasty, Submental Lipectomy, Erbium Laser Treatment of brown spot of right lateral face, and Revision of old scar of left jawline.

Comments: This 34 year-old soldier said he’d jumped out of planes and been shot at, but was never as nervous as he was on the day of surgery. An old scar of the left jawline was revised simultaneously. He did not find it necessary to take the prescribed painkillers and took Tylenol® instead. Eight days after surgery he was back in uniform. At the time of his visit nine days after surgery, he still had the expected degree of swelling and bruising. The brown spot on the right cheek was gone. When he returned home for Thanksgiving, he said only one of his two sisters noticed a difference in his appearance. His mother did not. He was told that he looked younger. He said that he ‘could not be happier” with the result.

Comments: This 37 year-old Hispanic male had always been self-conscious about his small chin and wore a goatee for camouflage, like so many men who have a small chin. He wanted a more Caucasian appearance.

Testimonial: "I had some breathing problems because of past sports injuries to my nose and I decided to have my upper eyelids done at the same time. The first two or three days were pretty miserable. After that, it really wasn’t bad. The difference in my appearance is subtle and natural-looking, which is what I wanted, and my breathing is better.”

Comments: This patient wanted a larger, more “Roman” nose. He also had nasal obstruction from a deviated septum. The septoplasty served two purposes – correction of the nasal obstruction and graft material to be used, along with cartilage from the left ear, to build up the nasal dorsum. His “bug-eyed” appearance was treated simultaneously with lower blepharoplasties and fat injection to the upper cheeks and tear troughs. His upper lip and nasolabial creases were also treated to give him a fuller lip and smooth his smile lines. He attended church 10 days after surgery and was back to work servicing heating and air conditioning systems 11 days after surgery.

J.L. (Age 52) - Medical Sales
Procedure: Rhinoplasty
Description: Chin/Jowl Augmentation, Sclerolaser™ Treatment of Facial Veins, Facial Scar Revisions, Submental Lipectomy, Fat Injection. Additional procedures, two years later: Facelift, CO2 laser resurfacing, Tip Rhinoplasty, Additional Scar Revisions and Fat Injection, Upper and Lower Blepharoplasties, Endoscopic Forehead Lift, and Buccal Fat Pad Resection.

Comments: This patient thought his face looked “pudgy.” He was treated with a combination of a tip rhinoplasty, submental lipectomy, chin/jowl augmentation and resection of his cheek fat pads. This helped give him a more chiseled profile.

R.K. (Age 59) - Retired
Procedure: Rhinoplasty
Description: Rhinoplasty, excision of excess right cheek fat, cautery of nasal veins.

Comments: This patient found his nasal tip a little too bulbous and droopy. He is seen only one month after his rhinoplasty (he still has some swelling of the tip). The tip of the nose is elevated so that the angle between his nose and upper lip is now 90 degrees, the preferred angle for men. His nose looks more refined, but not overly so. He reported very little pain and in fact did not take any painkillers after the date of surgery. He has limited swelling and bruising and was comfortable attending a wedding just a few weeks after his surgery. He was also glad that I had not packed his nose and that he could breathe through his nose easily the day after surgery.
